I, the undersigned volunteer, as being a staff member for the Junior Youth Camp, and my family members present acknowledge, and agree and understand that:
Voluntarily and of my own free will, I elect to participate as a member of the staff of the Junior Camp, and all the activities involved with the camp.
I, understand that there are certain risks, and hazards involved in participating in Junior Camp that may result in injury or even death to me or my family members. Including, but not limited to, those hazards
associated with the weather condition, playing condition, equipment and other participants.
I understand that by participating in the activities of Junior Youth Camp it may be dangerous to family members and me and to others and may result in a serious injury or even death.
I understand that in participating in all or in even some of the events of Junior Camp there can be and is some hazardous and some risky natures involved. Including, but not limited to , all the acts of the activities planned for the camp. All sporting events, the act of pitching, throwing, fielding and catching of the balls, the swinging of the bat, running, jumping, stretching, sliding, diving, and collisions, with
other players and with stationary objects, all of which can cause serious injury or even death to my
family or me.
Furthermore, I the undersigned player and staff member agree that upon consideration for the right to
play or be a member of the Junior Youth Camp Staff, which would also include all of those members of my family, and or those I will be signing for:
I voluntarily elect to assume all the risks of the injury incurred or suffered by my family or myself while participating as a staff member of the Junior Youth Camp, while involved in such activities of the Camp or being a spectator for the duration of this Camp.
I, release, discharge, and agree not to sue the Junior Youth Camp Committee, the Western District of the United Pentecostal Church International or any other staff member or entity connected with the
said organizations, for any claim, damage costs, cost or cause of action which I have or may in the future have as a result of any injuries or damages sustained or incurred by my family or myself from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.
I acknowledge that I have read and understood each and every one of the provisions in this waiver, and I fully agree to Abide by them.
